Phillips Model HS520, love them.
Fits over the ear/behind the head. Can wear with a hat, they even have a reflective strip across the back if that helps. Nice long cord when I attach my mp3 to my shorts behind my back. $21.99 at Best Buy, they do have cheaper ones but these have served me well.
I cant stand the ones that go over my ear so i went to the source and got ear buds that are almost like foamy ear plugs that you put into your ear. I found the stay tight and have good quality of sound. I believe they are made by Centrios.
i have some Sony MDREX85LPB which hace silicon, in the ear buds. they stay put when i run, and sound fantastic... long enough cord to comfortably reach the pocket on back of my running shorts... a little more expensive than basic earphones - £32.98 online, so maybe $50 ish going on conversion rates? although suspect they would be a bit cheaper in the states (anything electrical usually is)...
i wear a hat with them too, but incredibly comfortable and fantastic clarity of bass and treble...
I have a set of Aiwa HP-J30's that have been the best for me - they are the kind that go into your ear w/ the speaker facing forward and then wrap over your ears and behind your head. They fit well, sound pretty good (who needs hifi when running) and have lasted well.
I love Sony Earbuds, but the problem is I sweat too much and I flood them out and kill the speaker. I tried the Nike Vapor's but don't like them. They are a "ear clip" type but they wouldn't stay put - the speaker part is too big to fit tightly in my ear and the clip over the ear is too loose to hold them tight.

I have recently bought Sennheiser PMX-70's off amazon, they were £14/$27, which is well worth it in my opinion. They don't move around, don't get squelchy, waterproof,sweat-proof, over-ear clips and neckband. Feels weird at first and the music shots forward and not into your ears...but it works, the sound for me is great, and they suit the shapes of my ears.
Several pairs of headphones I have tried just fall out or are too uncomfortable for my ears.Everyones are different, which makes finding headphones hard. I would definately reccomend these. Not sure about the ones witout the neckband, or the plugs, they are similarly priced.
Phillips SHS3200, they run about $15.00 at Bestbuy.
Good sound, great price. Fit secure (if your heads not too small, a problem with the ear hook style)

I have finally found a great pair of Sennheiser in ear phones. They cost around £30 but well worth it. Quality is excellent and no falling out! they come with additional covers which will suit any shape of ear.
I brought mine from Innovations magazine but I'm sure you can get them on line.
as well as running i cycle to work and although for safety i shouldnt i do use headphones.....so will the ones with bands also fit on with a cycle helmet [Giro type]....
